Hospital Discharge to Assisted Living


Carolyn, age 82 and suffering from dementia, lives in an assisted living community where she has settled into a comfortable routine and seems happy. Carolyn was hospitalized for a fractured hip and the hospital discharge planner recommended short-term rehab at discharge. Carolyn became more confused at the hospital and might have become even more confused when she moved again to rehab. The family could not believe a rehab facility was the only option for receiving the care Carolyn needed. The family contacted Care Management Solutions for help. After speaking with the family, hospital staff and the assisted living nurse, the Care Manager proposed that Carolyn be discharged directly back to the assisted living with home therapy, home health aide services and increased care services from the assisted living staff while she recovers. The family is very pleased with the plan and Carolyn continues to thrive. The Care Manager checks on Carolyn periodically and remains available if her needs change in the future.

Meet the Care Manager


Cindy Dubansky, MSW, LICSW, C-ASWCM has over 30 years of experience as a social worker in a variety of health care settings including hospital, rehabilitation, dialysis, hospice and care management. She is skilled in the areas of care management, navigating the health care system, health care advocacy and client/family education.


Ms. Dubansky holds a Master of Social Work degree from The Ohio State University. She is licensed to practice social work in Minnesota and Iowa, certified to practice social work in Wisconsin, certified by the Academy of Certified Social Workers and is a Certified Advanced Social Work Case Manager through the National Association of Social Workers.


A highly regarded professional, Ms. Dubansky has presented nationally and locally on a variety of health care and psychosocial topics. She is active in professional organizations including the National Association of Social Workers, Minneapolis Area Senior Workers Association, Senior Workers Association of the Greater St. Paul Area, Minnesota Association of Home Care Social Workers, Memory Care Professionals and the Minnesota Health Literacy Partnership.
